According to data from the Federal Statistical Office (Destatis), the number of people working in German public health offices rose to 27,225 by the end of 2025. This represents an increase of 570 individuals, or 2.1 percent, compared to the previous year. The number of full-time equivalents reached 22,110, up by 365, or 1.7 percent, year-on-year. Both the total headcount and full-time equivalents grew more strongly in 2025 than in 2024, when the increases were 335 persons (plus 1.3 percent) and 275 full-time equivalents (plus 1.3 percent), respectively.
Growth in permanent staff was weaker in 2025 compared to 2024. The number of employees with indefinite contracts rose by 385 persons, or 1.5 percent, down from an increase of 800 persons, or 3.3 percent, in the prior year. In contrast, staff with fixed-term contracts grew by 190 persons, or 10.8 percent, after a sharp decline of 465 persons, or 21.1 percent, in 2024. By the end of 2025, 25,305 employees held permanent contracts and 1,920 had temporary contracts. Permanent tariff employees made up the largest group at 83 percent, followed by civil servants at 10 percent and fixed-term staff at 7 percent.
Just over half of all staff in public health offices—52 percent—were classified as highly qualified, meaning they held a university, master craftsman, technician, or technical school degree. This share varied regionally from 31 percent to 86 percent. Over the course of 2025, the number of highly qualified employees rose by 410 persons, or 3.0 percent.
At the end of 2025, the workforce composition was 20 percent physicians and dentists, 55 percent specialized technical staff, and 25 percent administrative personnel. Compared to the previous year, medical and dental staff grew by 195 persons, or 3.6 percent, and specialized technical staff increased by 400 persons, or 2.8 percent. Meanwhile, administrative staff declined by 20 persons, or 0.3 percent.
The average age of public health office employees nationwide was 47.7 years at the end of 2025. The share of workers under 35 ranged regionally from 6 percent to 25 percent, with a national average of 14 percent. The proportion of employees aged 55 and older averaged 34 percent nationally, varying between 17 percent and 47 percent across regions. The age distribution showed little correlation with region type—urban, intermediate, or rural.
Women accounted for 82 percent of the workforce in public health offices as of December 31, 2025. They were far more likely to work part-time, at 60 percent, compared to 23 percent of men. Overall, 46 percent of staff worked full-time and 54 percent worked part-time. The share of marginal part-time employees was below 1 percent. On average, 1.2 persons were employed per full-time equivalent, with regional variation from 1.1 to 1.5.
The largest share of working time in public health offices was devoted to child and youth health, accounting for 18 percent of all activity. Administration followed at 14 percent, and social psychiatry and mental health at 12 percent. Among physicians and dentists, child and youth health represented the largest work area at 30 percent, followed by adult assessment at 18 percent.
