An Erie primary-care medical clinic that catered specifically to Medicare patients has closed just two years after it opened.
Signs at the former Oak Street Health, 1535 W. 26th St., have been removed. A notice taped to the building’s front door said, “This location is permanently closed.”
“We made the difficult decision to close 16 Oak Street Health centers — including a center in Erie — earlier this year,” said Roslyn Guarino, a spokeswoman for CVS Health, the centers’ parent company.
CVS Health opened Oak Street Health in early 2024 after converting its CVS Pharmacy at that location into a hybrid pharmacy/clinic. The pharmacy, located on the east side of the building, remains in operation.
Oak Street Health specialized in preventative, personalized care for older adults, company officials said before opening the clinic.
“The (16 clinic closings) positioned Oak Street Health for sustainable, long-term growth as we continue to navigate external challenges, such as elevated medical costs, (Centers for Medicare & Medicaid Services) risk adjustment model changes, and health plan payer dynamics,” Guarino said.
The clinic’s staff were considered for jobs at other CVS Health locations, while Guarino said the company worked at helping patients find other providers. The nearest open Oak Street Health clinics appear to be in Cleveland, according to the company’s website.
