A notice taped to the building&#8217;s front door said, &#8220;This location is permanently closed.&#8221;<br \/>&#8220;We made the difficult decision to close 16 Oak Street Health centers \u2014\u00a0including a center in\u00a0Erie\u00a0\u2014 earlier this year,&#8221; said Roslyn Guarino, a spokeswoman for CVS Health, the centers&#8217; parent company.<br \/>CVS Health opened Oak Street Health in early 2024 after converting its CVS Pharmacy at that location into a hybrid pharmacy\/clinic. The pharmacy, located on the east side of the building, remains in operation.<br \/>Oak Street Health specialized in preventative, personalized care for older adults, company officials said before opening the clinic.<br \/>&#8220;The (16 clinic closings) positioned\u00a0Oak Street Health for sustainable, long-term growth as we continue to navigate external challenges, such as elevated medical costs, (Centers for Medicare &#038; Medicaid Services) risk adjustment model changes, and health plan payer dynamics,&#8221; Guarino said.<br \/>The clinic&#8217;s staff were considered for jobs at other CVS Health locations, while Guarino said the company worked at helping patients find other providers.
